Job Responsibilities: |
 |
 |
To assess and admit new cases |
 |
 |
To identify the social and financial needs of the new applications and conduct periodic assessment for on-going cases |
 |
 |
To administer the various subsidies and claims e.g. Medifund, IDAPE, Eldershield, MFEC, PA, other Financial and Social Aid |
 |
 |
To liaise with medical and nursing staff to enhance the patients’ medical, psychological and emotional health |
 |
 |
To update and control all clients’ record and case notes |
 |
 |
To provide financial counseling for patients and their families |
 |
 |
To help patients and their families cope better through psycho-emotional counseling |
 |
 |
To assist in project work |
 |
 |
To liaise with other social work agencies |
 |
 |
To identify and match relevant services available to the needs of the clients |
 |
 |
To offer support and/or assistance to needy households by identifying the needs and match relevant services to their needs |

Job Requirements |
 |
 |
Bachelor in Social Work or equivalent |
 |
 |
With 1-2 years’ experience working with institution or social Agency |
 |
 |
Possess computer skills in Microsoft Office, e.g. Word, Excel and PowerPoint, etc. |
 |
 |
Has good communication and interpersonal skill |
 |
 |
Enjoys working with different groups of people |
 |
 |
Experience in working with a VWO is an added advantage |
 |
 |
Applicants should be Singaporean citizens or hold relevant residence status |
 |
 |
